IMPORTANT INFORMATION FOR OUR DENTAL PATIENTS

Welcome to Uptown Park Dental. Here, Dr Dan Czerniecki and the rest of our team use their years of experience to help New Westminster smiles of all ages shine their happiest. Below is everything you need to know to join us. Contact us now to schedule an appointment or to learn more about our services.

NEW PATIENTS

We are happy to see new patients, and we appreciate the referral of your friends and family. For your initial visit, please arrive approximately 15 minutes before your appointment time in order to complete the forms we require prior to the treatment. It is important to provide information about your current and past medical history, any medications you are taking or have recently taken, and your dental insurance coverage. Our friendly staff can help you with filling out the forms. In order to assist in providing a baseline for your dental health and prevent the unnecessary duplication of X-rays, we request you to contact your previous dental office and arrange to forward your most recent dental radiographs.

DENTAL INSURANCE POLICY

Dental insurance is a contract between the patient, their employer or union, and the insurance company. We are not a party to that contract, but as a convenience for our patients we will work with most dental plans by processing claims for treatment and accepting payment directly from the insurance company for the portion covered by the plan. The wide variety of dental plans with varying benefit levels makes it impossible for us to know all the details about every patient’s coverage. It is the patient’s responsibility to understand their plan and inform us about any changes. Because the treatment is provided to the patient, not the insurance company, you are legally responsible for all costs that not covered by the dental plan. We will assist you in determining what your dental plan covers, and, can send in a predetermination request to the insurance company prior to commencing treatment. Frequently, dental plans send the predeterminations only to the patient not to us, so please advise us when you receive it. We can review this with you and provide a treatment estimate for you.

DENTAL FEES

Our fees generally follow the fee guide which is set by the BC Dental Association and is followed by most dental plans. Payment for your portion of our fees is due as treatment is rendered, and we accept Visa, MasterCard, Interac, and cash.

APPOINTMENT REMINDERS

We primarily use email and/or phone calls to remind our patients about their scheduled appointments and when they are due for their check-up and cleaning appointments. You will be asked to update this information whenever you come into the office. These are the only reasons for asking your email and phone contact information, and this information will not be shared with/sold to any other organization or company.

MISSED APPOINTMENT POLICY

While we will make every effort to confirm your appointment, it is ultimately your responsibility to remember and attend your appointments. If you are not able to attend an appointment, we require 48 hours’ notice, otherwise, a fee may be charged.
